I release Henderson Hockey Training and its Instructors from all claims for any damages arising from any accident/injury or loss which is caused by or arises from participation of the above named participant. I also understand that cancellations must be made 3 weeks in advance in order to receive a refund and that a $50.00 administration fee will apply.
